CRIPPLE CREEK, Colorado — The tragic death of a tour guide at the Mollie Kathleen Mine last October has been attributed to "operator error," according to the conclusion of a multi-agency investigation. Patrick Weier’s death was deemed accidental, with investigators confirming that no equipment malfunction or procedural issues contributed to the incident.

The investigation, conducted by Colorado’s Department of Labor and Employment and other agencies, examined safety protocols, equipment conditions, and operational procedures within the mine. Inspectors assessed various safety factors including ventilation, fire prevention, and personnel hoisting mechanisms. While the details surrounding the operator error were not elaborated upon, authorities have confirmed that the mine can resume operations now that the cease-and-desist order has been lifted.

During the incident, which occurred around 500 feet underground, Weier died, and 12 others were trapped for six hours before being rescued. The mine’s historical significance in the district, which has been conducting tours for approximately 60 years, is highlighted by the fact that it is not actively mining gold but rather operating as a tourist destination.

The mine’s website stated, "Recently the Mollie Kathleen experienced a tragic event," indicating that their closure would remain in effect until further notice. Even so, local officials noted that incidents of this nature have been rare, with no similar fatalities reported in nearly 40 years.

As of Wednesday evening, operators of the Mollie Kathleen Mine had not disclosed when tours would recommence, leaving community members and visitors hopeful for a safe reopening soon.
